Introduction to your child moving out and on
This section of the website is dedicated to helping you learn how to help your child transition and be able to live on their own.

Here are some question to ask yourself and your child to help you both develop ideas.
During your child's Individualized Education Plan (IEP) meeting at the age of 14 s/he should have had the "Statement of Transition Service Needs" added to the IEP. This would contain ideas on how s/he should be educated to prepare him/her for life outside of school.
At the age of 16 you child has "Needed Transition Services" added to his/her IEP. This has education and services needed to achieve the goals set out for the student after graduation. This can include things such as occupational therapy and workability.
In Contra Costa County we are also required by law to use the Individual Transition Plan (ITP) which contains information about the planning and development of the goals set in the IEP. These must always be developed with the parents as they are meant to integrate the school and home experience so that the student may work towards his/her goals in a holistic way.
